Forms

After you have been scheduled for a new patient appointment at either the Bellevue or Kirkland office, please review the corresponding welcoming letter below (these letters are identical except for the different street directions for each office).  We would be particularly appreciative if you could print out and complete (at your convenience) the Health Profile Form.  This form will expedite our review of your past medical history and allow us to spend more time during your appointment focusing on diagnosis and treatment.

(The documents below are in Adobe Acrobat format--if you happen to not have Acrobat on your computer it can be downloaded without charge at Adobe)  Please note that it is possible to type directly into (and tab between) the fields of the Health Profile Form (if needed, to activate this function just click on the "Highlight Fields" button in the top Right hand corner of this form).  We prefer that you either fax the form to us (at 425-899-6228) or bring it with you at the time of the appointment (rather than submit online) because of confidentiality issues related to transmission over the Internet.  We would, of course, be grateful for any suggestions as to how we might improve this webpage or any other aspect of our care.
